Please carefully note that there are three categories under which to apply. Category I is only for those applicants who practice in jurisdictions where licensure is not available. International applicants should contact the ABA for information on ABA requirements.
Category 1: Board Certification
For applicants who meet all requirements but practice in jurisdictions where licensure is not available.
- Application Form (form #1)
- Employment History Form (form #2)
- Professional Practice Verification Form (form #4)
- Notarized Verification of Score Report Form (form #5)
- Official doctoral transcript requested to be sent directly to ABA headquarters
- Fee
Category 2: Board Certification through Reciprocal Eligibility
For applicants who hold a state license/registration
- Application Form (form #1)
- Employment History Form (form #2)
- Copy of current state license/registration that includes expiration date
- Copy of Score Report from a National Examination in Audiology
- Official doctoral transcript requested to be sent directly to ABA headquarters
- Fee
